Job description

Our client, a Greater Manchester Council is seeking an Adults Advanced Practitioner to join their Urgent Care Team. Fantastic payrate of £32 per hour! Responsibilities of this role: • Lead on complex, high-risk and urgent adult social care cases, providing expert professional oversight. • Undertake advanced assessments, risk assessments and care planning for adults with complex needs. • Support crisis intervention, hospital avoidance and safe discharge planning. • Provide coaching, mentoring and practice guidance to Social Workers and newly qualified staff. • Act as a key source of professional expertise within the Urgent Care Team. • Support the Team Manager in driving high standards of practice and service delivery. • Chair or contribute to complex case discussions, professional meetings and multi-disciplinary forums. • Promote strengths-based practice, positive risk-taking and person-centred approaches. • Work closely with Health colleagues, Community Services, Housing and partner agencies to coordinate urgent interventions. • Apply expert knowledge of the Care Act 2014, Mental Capacity Act 2005 and safeguarding legislation. • Support quality assurance activities, case audits and continuous service improvement. • Ensure accurate and timely recording, report writing and statutory compliance. • Escalate risks appropriately and provide professional challenge where required. • Deliver positive outcomes for adults requiring urgent intervention, crisis support and care management.
