This is a new role within the Practice supporting the GP on the day with clinical triage and home visits Working closely alongside our GP team, the successful candidate will play a key role in managing acute on-the-day demand and supporting patients within the community. Key responsibilities will include: • Conducting home visits for housebound and clinically appropriate patients. • Managing on-the-day acute presentations. • Assessing, diagnosing and treating patients independently within their scope of practice. • Supporting and managing triage requests through our newly implemented Klinik online consultation and triage system. • Providing clinical advice and signposting patients to the most appropriate service. • Prescribing independently. • Maintaining accurate and timely clinical records. • Working collaboratively with GPs, practice nurses, administrative staff and wider healthcare professionals.
