Job description

Key Responsibilities • Provide autonomous remote clinical assessments via telephone and digital consultation. • Assess patients of all ages presenting with acute, chronic and non-acute conditions. • Take comprehensive clinical histories and formulate differential diagnoses and treatment plans. • Prescribe safely and appropriately within your scope of practice. • Refer patients to appropriate primary, community, secondary or emergency care services. • Recognise deteriorating patients and initiate emergency interventions where required. • Identify safeguarding concerns and take appropriate action in line with local and national guidance. • Ensure patients receive clear safety-netting advice and appropriate follow-up arrangements. • Deliver high-quality, person-centred care while meeting service performance standards. • Maintain accurate, timely and comprehensive clinical records. • Work collaboratively with the wider multidisciplinary team to deliver safe, effective patient care. Essential Requirements • Current registration with the NMC or HCPC. • MSc Advanced Clinical Practice (or equivalent), including an Independent/Non-Medical Prescribing qualification. • Minimum of 6 years post-registration experience, including: • At least 3 years in General Practice, or • 3 years within an Urgent Treatment Centre, Emergency Department or Integrated Urgent Care setting. • Level 3 Adult and Children Safeguarding. • Up-to-date mandatory training. • Eligible to work in the UK. • Enhanced DBS clearance. • Demonstrable experience of autonomous practice across the four pillars of Advanced Clinical Practice. • Excellent clinical assessment, communication and decision-making skills. Desirable Skills • Teaching or mentorship qualification. • Experience using Adastra, EMIS, SystmOne and Electronic Prescription Service (EPS). • Strong organisational and workload management skills. • Experience within NHS 111 or an Integrated Urgent Care Clinical Assessment Service.
