Advanced Practitioner

The Advanced Clinical Practitioner (ACP) will work as an autonomous clinician within the multidisciplinary general practice team, providing high-quality, patient-centred care to patients presenting with both acute and long-term health conditions. Working within their agreed scope of practice, the post holder will assess, diagnose, investigate, treat and appropriately refer patients, using advanced clinical decision-making skills to deliver safe and effective care. The ACP will manage a varied clinical caseload, including same-day urgent presentations, minor illness and chronic disease managements. They will contribute to improving patient access, reducing GP workload and supporting the delivery of high-quality primary care services. In addition to their clinical responsibilities, the ACP will contribute to service development, clinical governance and quality improvement initiatives, supporting the practice in delivering safe, effective and responsive healthcare. The post holder will work collaboratively with GPs, nurses, pharmacists and the wider multidisciplinary team, providing clinical leadership, sharing knowledge and supporting the development of colleagues where appropriate. The post holder will maintain professional registration with the NMC or HCPC, practise in accordance with the relevant professional code of conduct, and ensure their knowledge and skills remain up to date through continuing professional development. They will work within national guidance, local policies and the practice's clinical governance framework, recognising the limits of their competence and seeking advice or supervision when required. The role may also include participation in practice meetings, audit, education, service improvement projects and the supervision of students or other healthcare professionals, depending on experience and the needs of the practice.
