To improve the health of individuals in local community through; • identifying and engaging with individuals from target groups (as agreed with manager). • supporting individuals in learning how to make better health choices and supporting them in initiating and sustaining appropriate behavioural changes through a reduction in alcohol consumption. • helping people to find and use the right services; • recording and reporting activity and results; • identifying and informing manager about people you are worried about.
