Transitional Lead – Adult Social Care
Based in London, hybrid working
Length: Initial 6 months
We are looking for exceptional Transitional Lead to provide strategic leadership and oversight during a significant period of organisational transformation. This will play a critical part in embedding sustainable culture and practice change, strengthening professional leadership, and supporting the transition to a neighbourhood-based model of working.
Working collaboratively with partners across health, housing, voluntary and community organisations, you will help shape neighbourhood-based ways of working that build on community assets and improve outcomes for local people.
Key Responsibilities:
• Lead and embed strengths-based, person-centred and outcome-focused practice
• Coach, mentor and develop managers and practitioners to strengthen professional confidence and leadership.
• Enable the transition to neighbourhood-based working by strengthening partnerships across health, housing and the voluntary and community sector
• Promote co-production and build meaningful connections with community assets
• Ensure high-quality delivery of statutory Care Act duties
• Provide oversight of quality, performance and risk
• Promote approaches that maximise independence, prevention and positive risk-taking
• Support the effective deployment of financial and workforce resources
Requirements:
• You will be an experienced senior leader with a recognised professional registration, such as Social Work, Occupational Therapy or another Allied Health Profession
• Extensive knowledge of the Care Act 2014, including wellbeing, prevention and safeguarding
• A strong understanding of strengths-based practice and neighbourhood models
• Experience of leading culture change, service transformation and practice improvement
• For one of the Transitional Lead assignments, experience of hospital discharge social work is essential
