Job Description: Social Worker SE AssessmentOverviewWe are seeking a compassionate and dedicated Social Worker specialising in SE (Social and Emotional) Assessment to join our inclusive team. Responsibilities • Conduct thorough social and emotional assessments to identify client needs, strengths, and challenges. • Develop and implement individualised care plans that address unauthorised behaviours and promote well-being. • Collaborate with multidisciplinary teams, including healthcare providers, educators, and families, to coordinate holistic support. • Maintain accurate and confidential case records in compliance with organisational policies and legal requirements. • Advocate for clients rights and access to necessary services and resources. • Facilitate group sessions and educational workshops focused on social and emotional development. • Monitor and evaluate client progress, adjusting plans as needed to ensure effectiveness. • Participate in ongoing professional development and contribute to a culture of continuous improvement and inclusion. Qualifications • Degree in Social Work (BSW, MSW) or equivalent qualification. • Registration or eligibility for registration with a recognised professional social work body. • Experience in conducting social and emotional assessments, particularly related to unauthorised behaviour intervention. • Strong interpersonal and communication skills, with cultural competence and sensitivity to diverse populations. • Ability to work independently and collaboratively within a multidisciplinary team. • Commitment to upholding principles of diversity, equity, and inclusion in all aspects of practice. • Knowledge of relevant legislation, policies, and best practices in social work and safeguarding. • Excellent organisational skills and the ability to manage a caseload effectively. Day-to-Day • Engage with clients through home visits, interviews, and observation to gather assessment information. • Liaise regularly with colleagues and external agencies to ensure coordinated service delivery. • Document assessment findings and progress notes promptly and accurately. • Participate in case conferences and team meetings to review and plan interventions. • Provide support and guidance to clients and families to encourage positive behavioural change. • Attend training sessions and workshops to stay updated on assessment techniques and policies. • Promote a welcoming and supportive environment that respects diverse backgrounds and experiences.
