Job description

The post holder will manage referrals into the Home1st and Domiciliary SLT pathways, including triage, prioritisation and waiting list management. The post holder will assess, develop, implement and evaluate specialist Speech and Language Therapy interventions for adults with acquired communication and swallowing disorders. The post holder will provide specialist advice to patients, relatives, carers, non-specialist staff and other professionals regarding the management and care of adults with communication and swallowing difficulties. The post holder will involve clients, relatives and carers in planning and prioritising care plans and will contribute to multidisciplinary planning to support identified goals and outcomes.
