Job description

Provide continuous therapeutic observation for adults and CYP at risk of self-harm, suicide, aggression, absconding or exploitation, in accordance with Trust policy. Undertake and document ongoing mental state and risk assessments, recognising and escalating changes in presentation or level of risk. Maintain therapeutic engagement with patients during enhanced care to support emotional regulation, reduce distress and promote safety. Recognise and escalate safeguarding concerns in line with Trust safeguarding procedures, including concerns relating to vulnerability, abuse or exploitation. Work within the principles of the Mental Capacity Act, recognising when patients may lack capacity and escalating concerns appropriately. Recognise and respond appropriately to disturbed behaviour, using de-escalation techniques and escalating where required to maintain safety. Support the care of patients with Learning Disability, Autism, mental illness, delirium or dementia, including ensuring reasonable adjustments are implemented. Work collaboratively with ward teams, safeguarding teams, paediatric teams and mental health services to ensure safe and coordinated care. Ensure accurate, contemporaneous documentation of observations, risk assessments, care provided and escalation of concerns. Escalate concerns appropriately to the nurse in charge and relevant senior staff to ensure patient Page 1 of 2 safety.
