Identify, assess, and manage risk, including suicide, safeguarding, and vulnerability. Ensure risk assessments are current and accurately recorded. Work collaboratively within MDTs to support coordinated care delivery. Liaise with GPs, inpatient services, social care, housing, voluntary sector and other agencies.
