Job description

Main Duties • Lead proactive engagement with patients eligible for bowel, breast and cervical screening. • Identify patients overdue for screening and support them to access services. • Conduct telephone, SMS, letter and interpreter-supported outreach. • Support patients to overcome barriers to screening attendance using culturally sensitive and trauma-informed approaches. • Facilitate co-production activities involving people with lived experience. • Develop relationships with community organisations supporting underserved populations. • Act as a point of contact for patients requiring additional support. • Support Cancer Care Reviews and signpost patients to appropriate services. • Maintain project records, dashboards and activity monitoring templates. • Collect patient feedback, case studies and evaluation data. • Support quality improvement and implementation of sustainable screening processes. • Maintain accurate records using EMIS and other clinical systems.
