The Care Co-ordinator plays an important role in supporting the delivery of high-quality, patient-centred care. Working as part of the multidisciplinary team, the post holder will coordinate care for patients with complex, long-term or multiple health needs, ensuring they receive timely, appropriate, and well-coordinated support.
The role involves working closely with GPs, nurses, practice staff, community services, and external organisations to improve communication, reduce barriers to care, and enhance the patient experience.
