Job Description: Care Coordinator About us CareFocal is a CQC Registered Care provider based in Erith and East London, with operational areas mainly across Bexley, Dartford, Greenwich, Bromley, Lewisham, Stratford, Bow, Barking, Medway and Essex. We provide high-quality home care and community services to vulnerable Adult and Children. Summary of role: Care Coordinator The Care Coordinator is to generate and promote a caring and supportive environment for CareFocal’s clients by implementing very high standards of professional care and support. To manage the staff team, supporting their development, skills and abilities. Principal responsibilities: Care Coordinator The Care Coordinator will provide good service to clients by ensuring that prior to each service commencing, a risk assessment with the client, and/or their chosen representatives, has been completed including what the client needs and would like to achieve from their care and support. The Care Coordinator will implement quality management and improvement systems. Effectively manage complaints and incidents. Carry out investigations relating to the quality of the service and use findings to make improvements. The Care Coordinator will support Staff Recruitment, Induction and Training. Provide information, guidance and on-going supervision to enable staff to effectively and safely carry out their roles. Carry out appraisals and monitoring of staff performance The Care Coordinator will support with growing hours to support growth and development of the business. The Care Coordinator will demonstrable good understanding of CQC key lines of enquiries. The Care Coordinator will be responsible for Staff Rota Planning and Co-coordination. To ensure that there is good communication and effective team building amongst all staff, including holding regular team meetings. Explore new potential areas of growth and expansion. Aware and support compliance in line with CQC guidelines . Support with generating and developing new business opportunities. Networking and relationship building.
