Care Coordinator - Tonbridge Primary Care Network

Using population health intelligence and risk stratification tools, the post holder will proactively identify patients who would benefit from personalised care and support planning, helping people navigate health and care services, promoting independence and self-management, reducing health inequalities and supporting the delivery of the PCN's proactive care, Enhanced Health in Care Homes (EHCH) and frailty programmes. In addition, the Care Coordinator will support the operational delivery of PCN hub services by coordinating and maintaining clinic schedules, managing patient bookings, cancellations and rebooking where required, and providing administrative support for vaccination programmes and other NHS and PCN clinics.
