About the Opportunity We are seeking a proactive and experienced Care Coordinator to join our growing team. This pivotal role ensures our clients receive exceptional care and our support workers are well-supported and motivated. Key Responsibilities ‍ ‍  Client Care • Develop and manage care schedules tailored to individual needs. • Ensure continuity of care during staff absences. • Maintain strong communication with clients and families. • Lead team meetings to foster collaboration and problem-solving.  Staff Management • Recruit and match support workers to clients. • Work with HR and Clinical Leads to resolve staff issues. • Support ongoing training and development.  Communication & Collaboration • Liaise with internal and external stakeholders. • Supervise care and medication standards. • Maintain accurate records and report on client progress.  Operational Efficiency • Plan and deploy resources across care packages. • Assist with scheduling, billing, payroll, and on-call duties.
