Clinical Case Manager (RN) - UK Based working for an Australian Company | Full-Time Subcontractor Provide clinical oversight. Support safe care. Make a real difference. Fortify Health Care Services, part of Fortify Health Group, is a for-purpose health and disability organisation delivering specialist nursing, mental health and NDIS services across South East Queensland, Australia. We exist to resolve complexity and empower individuals to achieve improved health and wellbeing. We are seeking an experienced Registered Nurse to join our growing clinical team as a Remote Clinical Case Manager (CCM). This unique opportunity allows you to work remotely from the UK while supporting Australian disability and healthcare services during Australian overnight hours. Working closely with our existing team in the UK and Australian-based teams, you will provide advanced clinical oversight, risk management support, governance activities and clinical escalation advice. This is a non-frontline role focused on clinical judgement, quality assurance and safe service delivery. About the Role As a Clinical Case Manager, you will: • Provide overnight clinical on-call support and escalation advice to frontline teams. • Conduct clinical triage, risk assessments and incident reviews. • Review care plans, clinical assessments, medication documentation and training plans. • Analyse trends, identify emerging risks and support clinical governance activities. • Prepare clinical reports, risk summaries and documentation supporting NDIS-related processes. • Collaborate with Specialist Care Coordinators and Australian-based clinical teams to promote safe, evidence-based care. • Contribute to continuous improvement, audit activities and quality initiatives. On-Call Requirement This position includes participation in an on-call roster, approximately one in every three weekends, providing clinical support and escalation guidance as required. About You To be successful in this role, you will have: Essential • Bachelor of Nursing or equivalent qualification. • Current registration with the UK Nursing and Midwifery Council (NMC). • Ability and willingness to obtain AHPRA registration. • Experience in disability, community health, mental health, rehabilitation or complex care environments. • Strong written communication and clinical documentation skills. • Excellent critical thinking, risk assessment and decision-making abilities. • High level of IT literacy and confidence using digital clinical systems. • Ability to work independently in a remote environment. Desirable • Postgraduate qualifications in mental health, disability, rehabilitation or leadership. • Experience supporting people living with disabilities or working within comparable healthcare frameworks.
