Are you a dedicated, experienced nurse who always strives to give the highest standard of care to your patients? Do you have a particular interest in palliative care? We are seeking an experienced and motivated nurse to join this specialist palliative end-of-life community service. This is an exciting opportunity to be part of a professional, specialist palliative care team that will work alongside members of the wider multidisciplinary team of health and social care colleagues, both in the community and the acute care sectors. This model of care integrates the available services within the community into a one-team approach to provide the best possible care for all patients who are diagnosed by their GP as palliative, regardless of diagnosis or complexity, guided by the individual needs and wishes of the patient and their loved ones. This service will provide continuity of care throughout the patients journey, enabling people to die in their preferred place, which is often their home. You will need to have experience in caring for palliative care patients and preferably have Community nursing experience. You will have the opportunity to learn and gain experience through clinical reflective practice and continued professional development. The successful applicants will have good I.T. skills, be comfortable using digital technology, be of a flexible and adaptable personality, and be willing to embrace change. The post-holder must hold a current driving licence and have access to appropriate transport, as the post predominantly involves working independently to maximise place-based care. There is currently a part-time Band 6 Integrated Palliative Care Nurse Specialist position available in our Axminster and Seaton Services.
