Citrus Health Primary Care Network is recruiting for a Clinical Pharmacist to be part of the new Primary Care Network multidisciplinary team. The successful candidate will be working at Portway Family Practice to provide: • Prescribing quality improvements and medicines optimisation. • A holistic, personalised, patient-centred approach to care within the PCN community. • Support to the wider multidisciplinary team and proactive population health education. Background Citrus Health Primary Care Network delivers healthcare services to a population of 45,228. This is a unique opportunity to help co-create the future of the pharmacy workforce within the Primary Care Network. The successful candidate will have a higher degree in pharmacy and be a registered GPhC Independent Prescriber. They will also be expected to demonstrate completion of the national CPPE ARRS training pathway. The postholder will undertake face-to-face medication reviews for patients with polypharmacy, particularly older people, care home residents and those with multiple co-morbidities. The role includes managing long-term conditions, working directly with patients to assess and treat conditions, and promoting self-care. The successful applicant will work as part of a multidisciplinary team to develop and run processes for repeat prescription reauthorisation, medicines reconciliation following transfer of care, and systems to support safer prescribing. The successful candidate will have excellent written and verbal communication skills, experience of influencing others, and expertise in conducting clinical medication reviews. They will also provide leadership in quality improvement and clinical audit, while supporting delivery of the Quality and Outcomes Framework (QOF) and PCN Network Contract requirements. Experience working in General Practice or a Primary Care Network is preferred.
