. Clinical Safety Project Leadership & Planning
Facilitate the planning, coordination and delivery of Trust-wide digital clinical safety activities across multiple systems and programmes.
Develop and maintain integrated clinical safety delivery plans, timelines and trackers aligned to programme and deployment schedules.
Coordinate safety activity across system lifecycles, including configuration, testing, deployment, optimisation and retirement.
Proactively identify delivery risks, capacity issues or dependencies and escalate appropriately.
Operate with autonomy to prioritise workstreams and adjust plans in response to changing programme needs.
Manage delegated resources for delivery of clinical safety assurance activities (e.g., coordinating assurance support, scheduling workshops, and prioritising activity within agreed capacity), ensuring delivery to time and within agreed constraints.
Coordinate utilisation of delegated budgets for clinical safety training, assurance activity and external support, ensuring value for money and delivery against plan.
2. Clinical Safety Assurance & Documentation Management
Lead the coordination, quality assurance and control of:
o Clinical Risk Management Plan and Strategies
o Clinical safety Cases
o Hazard logs
Ensure documentation is complete, version-controlled, proportionate and reviewed and amendment throughout the life cycle of the healthcare system.
Ensure documentation is fit for internal and external assurance.
Maintain the Trusts digital system register, clearly evidencing clinical safety status and assurance maturity.
Coordinate Trust-wide DCB0160 deployment assurance for all digital system changes.
Ensure Clinical Safety Facilitators, throughout the divisions, are trained, supported and compliant.
Maintain the clinical safety training compliance register and the design and maintenance of the clinical safety communication platforms.
Collate and quality-check supplier DCB0129 compliance documentation, highlighting gaps or risks to the CSO.
3. Risk, Issue and Incident Management
Facilitate the management of digital clinical safety risks and issues, ensuring accurate recording, tracking and escalation.
Record, manage and track digital-related patient safety incidents in LFPSE / Datix in line with PSIRF.
Coordinate root cause analysis and digital safety investigations, ensuring actions are robust, proportionate and tracked to completion.
Escalate overdue or deteriorating risks in line with agreed governance thresholds.
Provide structured summaries of incident themes and learning to inform proactive risk management.
4. Stakeholder Engagement & Training
Lead and facilitate highly complex and sometimes contentious discussions regarding digital clinical safety risks, mitigation options, and deployment readiness where there may be resistance to change.
Deliver formal workshops and training to large groups of staff (clinical, digital, operational) to secure engagement and compliance with clinical safety processes.
Communicate and escalate sensitive patient safety issues arising from digital incidents or hazards, ensuring shared understanding and agreement on corrective actions.
Act as a senior point of contact for digital clinical safety matters for internal teams and system suppliers.
Plan and deliver clinical safety training and awareness sessions, embedding DCB0129/0160 principles into digital programmes.
Coordinate and support regular multidisciplinary Digital Clinical Safety Huddles and assurance meetings.
Promote a strong culture of digital clinical safety, openness and learning across clinical, digital and supplier communities.
Lead the development, implementation and continuous improvement of the Trusts Digital Clinical Safety Operating Model, including procedures, templates, training requirements and assurance controls.
Propose and implement Trust-wide service changes to improve clinical safety assurance performance (e.g., hazard management, documentation quality, training compliance, incident learning), with impact beyond the immediate team/programme.
5. Monitoring, Reporting & Governance
Inform and support the production of high-quality reports and dashboards summarising:
o Open hazards and mitigations
o Incident activity and trends
o Training compliance
o Assurance status across systems
Prepare briefing papers and assurance updates for governance groups, programme boards, audits and inspections.
Interpret complex safety information and performance data, compare response options, and recommend proportionate interventions to the CSO and programme leadership.
Monitor safety delivery milestones and escalate slippage or emerging risks.
Interpret complex safety information to identify trends, prioritise action and recommend intervention.
6. Service Improvement & Operating Model Development
Lead development and implementation of the Digital Clinical Safety Operating Model and associated procedures, templates and training requirements.
Drive performance improvement across clinical safety assurance (quality, timeliness, compliance), identifying bottlenecks and implementing changes.
Facilitate collaborative working across clinical, digital and programme teams to embed consistent assurance practice across the Trust.
