Job description

The post holder will deliver specialised dietetic services to patients, using a wide range of appropriate and sensitive communication skills and counselling, and behaviour change techniques. Frequently dealing with a wide spectrum of medical conditions and, therefore, nutritional diagnoses and dietary interventions The post holder will work as part of a multidisciplinary team providing dietetic input, liaising with other disciplines and agencies as required in planning and implementing patient care. The post holder will maintain accurate and detailed records with regards to patients as required both internally and with other services and compliant with local and professional record keeping standards including automated information systems in your work in order to improve quality and co-ordination of services and be responsible for the quality of data you produce. Carries out requests for work in primary care settings that arise to support the wider work of the department, and gives support as required to the development of nutrition services that address health inequalities. The post holder will support guidelines and policies that are developed to promote safe nutritional care and meet CQC and the Trust quality and safety agenda. The post holder will support the development of quality standards and procedures within the department, and participate in regular audit of service against departmental and Trust quality standards. The post holder provides education sessions to staff including nursing, medical and other AHP Staff, catering teams and patients.
