Community Healthcare Support Worker

Clinical: Under the supervision of the Registered clinicians; Community Health care assistants will support the implementation of care plans and provide information for assessments.Support patients with all elements of the nursing process and be able to assist the Registered clinicians in the on-going assessment, planning, management and evaluationof care.Perform and record clinical observations.Undertake clinical tasks appropriate to competence and setting, for example;venepuncture, ECGs and feedback findings to the Registered cliniciansAccurately record nutritional and fluid intake.Ensure the privacy, dignity and safety of patient is maintained at all times.To plan and organise ones own activity, prioritising workload appropriately in order to make the best use of time and feeding back to the team as appropriate.To input all activity with the service users onto Clinical Systems or equivalent data in order to enable the collection of accurate statistics, as required by the Trust.Recognise issues relating to safeguarding vulnerable children and adults to report any problems or raise concerns to the appropriate registered care professionals.Assist in the implementation of appropriate action to meet the specific physical, emotional, psychological, social cultural and spiritual needs of patients and their carers.Demonstrate an understanding of caring for patients with particular conditions for example mental illness, learning disabilities and autism.Under the direction or discussion with Registered Clinicians signpost patients to serviceswhich may be of benefit to their wellbeing, promoting independence and capability with their own network and community.Under the direction or discussion with the Registered Clinicians signpost patients to services which may benefit of their wellbeing, promoting community connections and make the best use of local asset.Professional:Demonstrate the ability to recognise changing priorities seeking advice and guidance from the Registered clinicians.Demonstrate understanding of all elements of the nursing process and be able to assist the Registered clinicians to gather information in relation to on-going assessment, planning, management and evaluation of care.Report back and share information with Registered clinicians on the condition, behaviour, activity and responses of patient, ensuring any immediate action to maintain the safety of the patient is in place.Assess the environmental risks before providing and implementing care within community settings.Demonstrate good understanding of the principles of consent.Demonstrate good understanding of the Mental Capacity Act / Mental health Act, Deprivation of Liberties and apply Best Interest principles to everyday practice. Leadership:Demonstrate skills in relation to coaching/teaching patients/carers and other staff.Under supervision of the Registered clinician assist with the development, implementation and monitoring of clinical standards and outcomes.Develop a working knowledge of other providers resources and referral systems to ensure patients needs are met, within parameters of practice.Communicate with patients, carers, professional colleagues and the voluntary and charitable sector to ensure seamless care is achieved.
