Job description

Community Mental Health Team – Social Worker Neath Port Talbot County Borough Council Based at the Civic Centre in Neath, this is a great opportunity to join a supportive Community Mental Health Team working with adults across the locality. You'll manage a caseload of more complex work, carrying out assessments that identify outcomes in partnership with individuals, families and carers. The role includes capacity assessments, Best Interest Assessments, CHC assessments and financial assessments within the Council's charging framework. You'll work jointly with other professionals and agencies, attend and contribute to case conferences and reviews, and take appropriate action to protect people at risk. AMHP duties may form part of the role where relevant. Work pattern: 4 days office based, 1 day from home (dependent on staffing levels).
