Aspects of the role include: To assess, plan, implement, promote and evaluate safe, creative and effective treatment plans in collaboration with the patients diverse needs, their carers and other professionalsDeliver interventions that are evidenced basedBe responsible for the safe monitoring, management and administration of medicationWork autonomously, building leadership skills and advanced clinical assessment skillsYou will build effective relationships with patients, their carers, family and colleagues across the MDTYou will develop exceptional planning and organisational skillsYou will be able to analyse care and treatment provided, and develop sound clinical decision makingYou will be trained to deliver high quality care and develop Knowledge in all areas including palliative care, leg ulcer management, acute and chronic wound care, understand bladder and bowel dysfunctions, provide supportive care to carers and family membersYou will delegate and support junior staff, building leadership skills A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. We would offer you: Clinical and managerial supervision.Full induction and training.Competitive mileage rates.No night shiftsHealth and wellbeing service including but not limited too, occupational health, MSK services, staff counselling and support services.A range of flexible working optionsIncentive schemeschildcare scheme, lease car, and travel pass schemeCareer progression and future secondment onto the Community Specialist District Nurse Programme Please note that this post does not meet the pay or skill level required for a Skilled worker visa. Successful applicants with no prior NHS experience would normally be placed at thebottom of the band in line with Agenda for Change.
