Job description

• Provide consultant-led clinical care and leadership across the Acute Medicine Service, including the Acute Medical Unit (AMU) Same Day Emergency Care (SDEC), and Hospital at Home (George Eliot @ Home) • Deliver timely senior clinical assessment and decision-making to support early diagnosis, treatment, admission avoidance and safe discharge. • Undertake consultant ward rounds, multidisciplinary board rounds and review of patients within Acute Medicine and SDEC. • Support the development of Same Day Emergency Care (SDEC) pathways and work collaboratively with George Eliot @ Home service to identify patients suitable for admission avoidance and early supported discharge. • Work closely with the Emergency Department, Frailty Assessment Unit, Planned Ambulatory Care, Gastro Day Unit, inpatient specialties and community teams to provide integrated, patient-centred care. • Contribute to the ongoing development of Acute Medicine services, ensuring pathways reflect national best practice and local service priorities. • Participate in the Acute Medicine and General Internal Medicine (GIM) on-call rota. • Participate in the development and implementation of clinical guidelines, audit, quality improvement initiatives and all aspects of the clinical governance agenda. • Demonstrate flexibility in undertaking appropriate clinical duties as requested by the Clinical Lead to meet service requirements. • Actively contribute to undergraduate and postgraduate education, providing supervision, teaching and support for doctors in training, advanced clinical practitioners and the wider multidisciplinary team. • Maintain and, where appropriate, develop a specialist clinical interest that complements the needs of the Trust and Acute Medicine Service. • Develop and maintain effective working relationships with General Practitioners, community providers and external partner organisations. • Comply with all Trust policies, procedures and statutory requirements, including Health and Safety, Information Governance, Data Protection, Incident Reporting and Standing Financial Instructions. • Maintain high standards of clinical practice through participation in Clinical Governance, Risk Management, audit and service evaluation. • Take responsibility for the educational supervision, appraisal and professional development of trainee doctors in accordance with national guidance. • Maintain Continuing Professional Development (CPD), participate in annual appraisal and support the Trust's revalidation process in line with Royal College of
