Continuing Healthcare Nurse Assessor (Band 6)
Location: Lancashire and South Cumbria (Blackpool, Preston, Blackburn, Hyndburn, Wyre, West Lancashire & South Lakeland)
Pay rate: £21.57 per hour
Hours: Full-time (37.5 hours, Monday to Friday) orpart-time (24+ hours considered)
Contract: Flexible Bank / temporary placement

As a continuing healthcare nurse assessor, you will play a key role in ensuring patients receive the right care and funding support at the right time.
Working across a range of settings, you will assess complex patient needs, contribute to funding decisions and support high-quality, fair and consistent care planning.

What youll be doing
• Carry out assessments for NHS continuing healthcare and funded nursing care
• Gather and analyse clinical evidence to support Decision Support Tools (DSTs)
• Work with multidisciplinary teams to make eligibility recommendations
• Present cases for panel decision-making
• Maintain accurate, contemporaneous patient records in line with NMC standards
• Build effective working relationships with NHS, local authority and partner organisations
• Support consistent, high-quality assessment practice across services
What were looking for
Essential:
• Registered nurse with valid NMC registration
• Experience working in continuing healthcare (ICB or CCG)
• Understanding of NHS Continuing Healthcare Framework and guidance
• Experience supporting patients with complex needs
• Strong assessment, communication and organisational skills
Desirable:
• Degree-level qualification (or equivalent experience)
• Experience working across health and social care organisations
• Knowledge of safeguarding and relevant legislation
