The post holder will collaborate with stakeholders in order to achieve agreed objectives and deliver Care Community specific initiatives associated with improving all stages of the patient journey. Day to day duties will be associated with implementing changes and delivering significant and sustainable improvements within the services through collaboration with stakeholder organisations.
