Diabetes Enhanced Nurse Practitioner

Manage a specialist caseload of patients with diabetes across inpatient and outpatient settings, providing high-quality, evidence-based care tailored to individual patient needs. Undertake autonomous, independent clinical practice under the direct and indirect supervision of the Senior Enhanced Practitioner within the diabetes service, using enhanced clinical judgement and decision-making skills to manage complex situations safely and effectively. Assess, plan, implement and evaluate care for patients with complex diabetes needs, supporting effective treatment plans and positive patient outcomes. Provide specialist diabetes advice, education and emotional support to patients, families and carers, empowering individuals to self-manage their condition and make informed decisions about their care. Initiate, review and modify diabetes interventions and treatment plans within scope of practice and Trust guidelines, including supporting the adjustment of insulin and oral diabetes medications. Work collaboratively with the wider multi-disciplinary team to ensure coordinated care pathways, safe discharge planning and effective ongoing diabetes management. Act as a clinical resource within the diabetes service, promoting evidence-based practice and supporting high standards of patient-centred care. Maintain accurate, comprehensive and contemporaneous clinical records in accordance with Trust policies and professional standards. Participate in diabetes education and training programmes for healthcare professionals and contribute to creating a positive learning environment within the service.
