Diabetes Specialist Nurse

You will help patients understand their glucose patterns, build confidence in self-management, use technology safely and make informed decisions about their diabetes care. You will work within a supportive, consultant-led service alongside colleagues across Spirit Health, GP practices and wider NHS teams. What youll do You will: • Review glucose data and wider clinical information. • Provide clear, personalised diabetes education and advice. • Support safe medicines optimisation and prescribing within your scope of practice. • Identify and escalate clinical concerns appropriately. • Maintain accurate clinical records and communicate clearly with GP practices. • Contribute to a safe, compassionate and practical service for patients.
