Dietetic Assistant Practitioner Community Nutritional Support

All Patient records are electronic whether it be Cerner as an inpatient or EMIS in the community. Good It skills are therefore required. The post holder will also be expected to: - use existing clinical pathways to assess, set patient centred goals, create treatment plan, monitor and evaluate a designated caseload of patients under the supervision of a Dietitian. - provide clinical care to a caseload of patients as specified by the Dietitian /Dietetic service working within the limits of your competencies under regular supervision in a variety of setting including individually or in groups. - advise, support and educate patients and carers in the self-management of the patients condition. The post holder will be required to communicate effectively with a range of healthcare professionals, patients,carers and family members. - liaise effectively with other departments/services /agencies, including information relating to patient interventions and service delivery. - follow agreed clinical pathways and interventions and escalate to Dietitian / supervisor when variations are noted or required. - make decisions within the scope of competencies and following clinical pathways. Workload will be delegated by Dietetic Clinical Leads based on competencies.
