Discharge Coordinator

This is a rare an exciting opportunity for an experienced nurse to join our Discharge Coordinator team which is part of the Transfer of Care Hub. As a Discharge Coordinator you will be responsible for the assessment and discharge planning of patients with complex discharge needs including those that are nearing the end of their life and are in the terminal phase of their illness. You will work as part of multi-disciplinary, multi-organisation team to ensure safe and timely discharges for this patient group.
