We are seeking an enthusiastic Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) to join our friendly, supportive and highly collaborative multidisciplinary team. The RoleAs an ACP or ANP, you will play a key role in delivering high-quality, patient-centred care across a broad range of clinical presentations. You will assess, diagnose, treat and manage patients presenting with acute illness, urgent care needs and long-term conditions, working both autonomously and collaboratively within our experienced clinical team. You will have the opportunity to utilise and further develop your advanced clinical skills while working alongside GPs, nurses, pharmacists, care coordinators and other healthcare professionals. We actively encourage innovation, clinical leadership and continuous service improvement, ensuring our patients receive the highest standards of care. As a teaching and training practice, we are committed to supporting clinicians at all stages of their careers and have a strong track record of helping ACPs, ANPs and other advanced practitioners develop their skills, confidence and areas of specialist interest. Key Responsibilities • Independently assess, diagnose and manage patients with acute and chronic health conditions. • Develop and implement evidence-based management plans. • Prescribe safely and effectively within your scope of practice. • Deliver health promotion, preventative care and patient education. • Manage same-day urgent presentations as well as routine clinical appointments. • Work collaboratively with the wider multidisciplinary team to deliver outstanding patient care. • Contribute to clinical governance, quality improvement projects and service development initiatives. • Support, mentor and share knowledge with colleagues where appropriate. What We're Looking For • Qualified Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP). • Current registration with the appropriate professional body. • Excellent clinical assessment, diagnostic and decision-making skills. • Strong communication, interpersonal and teamworking abilities. • A commitment to delivering safe, effective and compassionate patient care. • Independent Prescribing qualification desirable. • Experience in Primary Care is will be highly advantageous.
