Main duties & responsibilities:
1.Effectively manage and action all referrals received in a timely manner.
2.Ensure accurate and timely recording of all patient, carer, and professional contacts within SystmOne.
3.Take a central role in the day-to-day co-ordination of patient contact using our clinical noting system ensuring deadlines are met.
4.Coordinate seamless patient transitions from admission through to discharge within the Virtual Ward.
5.Maintain strong working relationships with internal teams, external partners, and relevant agencies.
6.Identify patients requiring complex discharge planning at an early stage and facilitate appropriate pathways.
7.Communicate clearly and compassionately with patients, families, carers, colleagues, and members of the public.
8.Develop and maintain an advanced working knowledge of SystmOne, supporting system improvements, reporting, and efficiency initiatives (including KPIs and activity reports).
9.Support audit processes and act as a Tendable champion for the service.
10.Manage general enquiries, escalating clinical or operational concerns appropriately.
11.Promote a positive, collaborative team culture that encourages continuous learning and service improvement.
12.To carry out noncomplex telephone contact with patients and families. This may include bereavement calls.
13.Using various methods of observation, report any change in the patients condition to the appropriate Health Care Professional.
14.Support in monitoring the lone working within the team.
15.Undertake additional duties to broaden skills and knowledge, as agreed with the line manager.
16.Assist the Clinical Nurse Specialists or Specialist Paramedics with joint visits as necessary and carry out independent home visits if required.
17.To ensure patient confidentiality is always maintained and information obtained during the course of employment is not released to anyone other than those acting in an official capacity.
18.Attend mandatory training on principles and core elements of palliative care.
19.To recognise and manage the potentially distressing effects of working with terminal illness, maintaining personal resilience while accessing the support, guidance, and resources available through the hospice.
