GP - Enhanced Access GP - Evening and Weekends

FOR EOI IN THIS ROLE CONTACT The General Practitioner will provide high-quality, patient-centred care as part of the Enhanced Access and Same Day Access Service, working collaboratively within a multidisciplinary team to deliver timely and effective primary care services to patients registered with E4 PCN member practices. Key responsibilities include: • Undertake face-to-face, telephone and remote consultations for patients booked through member GP practices, NHS 111 and Urgent Care Centre (UCC) pathways. • Assess, diagnose and manage a broad range of acute and routine primary care presentations, providing safe, effective and evidence-based treatment. • Provide comprehensive and competent assessment, clinical advice and treatment for primary care conditions, following national and local clinical guidelines and best practice standards. • Maintain accurate, timely and comprehensive clinical records of all patient consultations using the relevant clinical systems. • Request, interpret and act upon investigations as appropriate, including pathology and diagnostic tests. • Prescribe medications safely and appropriately in accordance with national and local prescribing guidelines, formularies and policies. • Initiate and manage referrals to secondary care, community services, diagnostic services and other healthcare agencies as clinically appropriate. • Ensure appropriate follow-up arrangements are made and communicate relevant information to patients and their registered GP practice. • Advise patients regarding the scope and limitations of the Enhanced Access and Same Day Access Service and signpost or direct them to alternative services where appropriate. • Manage clinical risk effectively, recognising and responding appropriately to urgent, emergency and safeguarding concerns. • Work collaboratively with GPs, Advanced Clinical Practitioners, Nurses, First Contact Physiotherapists, Pharmacists and other members of the multidisciplinary team to deliver integrated patient care. • Provide clinical support and advice to colleagues where appropriate and contribute to effective multidisciplinary team working. • Participate in clinical governance activities, including significant event reviews, audits, quality improvement initiatives and service development. • Maintain professional registration, appraisal, revalidation and mandatory training requirements. • Comply with all organisational policies, information governance requirements, safeguarding procedures and infection prevention and control standards. • Maintain appropriate professional standards in accordance with the General Medical Council (GMC) Good Medical Practice guidance and any locally agreed clinical standards and protocols. • Contribute to the ongoing development and improvement of the Enhanced Access and Same Day Access Service to ensure high standards of patient care and service delivery.
