• Build and maintain strong links with voluntary, community, and neighbourhood groups to map and maintain a directory of local assets, enabling timely, appropriate, and supported referrals. • Collaborate with local partners to strengthen and sustain community-based services and groups. • Engage in ongoing professional development, actively reviewing and enhancing the scope of the role in line with evolving community needs. • Provide tailored advice and support to individuals with memory concerns, ensuring early engagement and access to appropriate services. • Deliver emotional and practical support to patients and families throughout the dementia diagnosis process and as the condition progresses. • Act as a consistent point of contact, ensuring continuity of care and fostering trusted relationships with patients and carers. • Support carers through signposting, emotional support, advocacy, and by facilitating access to relevant services. • Assist veterans and their families in navigating and accessing appropriate healthcare and community-based services. • Promote self-management and independence by providing personalised support that enables individuals, families, and carers to take control of their health, make informed decisions, and improve health and wellbeing outcomes.
