JOB DESCRIPTION Job Title: GP Partner Hours: 48 sessions per week (flexible) Location: Polden Medical Practice, Edington and Woolavington, Somerset Responsible to: GP Partners Contract Type: GP Partner appointment subject to a 12-month mutual assessment period, following which progression to full equity partnership will be considered by mutual agreement Job Summary Polden Medical Practice is a forward-thinking rural dispensing training practice serving approximately 11,000 patients across two sites in the Somerset Polden Hills. We are seeking an enthusiastic and motivated GP to join our partnership team for 48 sessions per week. The successful candidate will provide high-quality, patient-centred general medical services while taking an active role in the leadership, development and future direction of the practice. This role offers the opportunity to become a genuine long-term equity partner within a stable and supportive partnership structure. Main Duties and Responsibilities Clinical Responsibilities • The post holder will: • Provide comprehensive primary care services to registered and temporary patients • Undertake routine, acute and complex consultations • Diagnose and manage acute and chronic medical conditions • Deliver evidence-based care in accordance with national and local guidance • Manage patients with long-term conditions and contribute to QOF achievement • Prescribe safely and effectively within professional guidelines • Participate in same-day access and triage services • Maintain accurate and contemporaneous clinical records • Make appropriate referrals to secondary care and community services • Work collaboratively with the wider multidisciplinary team to ensure high-quality patient care Same Day Access and Triage • The practice operates a fully digital same-day access system using Rapid Health • The post holder may be required to: • Review and clinically prioritise same-day patient requests • Support safe and effective patient navigation and care pathways • Work closely with Advanced Nurse Practitioners and the wider clinical team • Ensure appropriate escalation and management of urgent clinical presentations • Contribute to effective demand management within the practice Partnership, Leadership and Business Responsibilities • The post holder will: • Take an active role in practice leadership and decision-making • Contribute to the strategic direction and future development of the practice • Participate in partnership meetings and governance processes • Help shape service development and quality improvement initiatives • Support the delivery of safe, effective and sustainable primary care services • Participate fully in the management and running of the business • Work collaboratively with partners, managers and the wider practice team to ensure organisational success • Work collaboratively with the Primary Care Network, community services, secondary care providers and other stakeholders to support the delivery of high-quality patient care and the ongoing development of services Clinical Governance The post holder will: • Participate in clinical governance activities • Contribute to audit, quality improvement projects and service development • Participate in significant event analysis and learning • Promote a culture of continuous improvement and patient safety • Maintain professional registration and continuing professional development requirements Teaching and Team Working • The post holder will: • Work collaboratively within a multidisciplinary team • Support the training, supervision and development of staff where appropriate • Participate in clinical meetings and protected learning events • Contribute to an open, supportive and learning-focused practice culture Practice Profile Polden Medical Practice is a stable, well-established rural dispensing training practice serving approximately 11,000 patients across two sites.
