Job description

The post holder will also be required to undertake home visits, where appropriately trained and delegated, and work flexibly across all PCN member practices and care home settings to meet the needs of the service. The ability to travel between sites and work across multiple locations is therefore an essential requirement of the role. The role will have a particular focus on supporting: • People living with frailty • Care home residents • Housebound patients • Patients requiring proactive care • Individuals at risk of hospital admission • Patients requiring coordinated care across multiple health and care services. The Health Care Assistant will act as a central point of contact for patients, carers, GP practices and partner organisations, improving continuity of care by coordinating services across primary care, community services, secondary care, adult social care and the voluntary and community sector Please see attached job description and person specification for full details.
