Health & Justice Support Time Recovery worker

1. Working in a Health & Justice service in Worcestershire Outreach, with the flexibility of working across Worcestershire Health & Justice Services, dependent on skill and service need.2. Delivering a high quality short term intensive support service to patients and their families.3. Working towards developing a short term plan at the point of referral; this may be within Custody or Court settings, or within the community. The plan will be based on needs identified during assessment by Health & Justice Practitioner in Custody or Court. The aim of the intervention is to support and empower the individuals to reach identified goals and attend initial appointments with identified service. This will involve the individual in planning and prioritising their needs.4. Will be responsible for liaising with multiple agencies, criminal justice system and other partners.5. Identify, assess and refer people of all ages with a wide range of mental health, learning disability, substance misuse and/or social vulnerabilities when they first come into contact with the youth and adult criminal justice systems on suspicion of having committed a crime.6. When a person is assessed as having single or multiple vulnerabilities, they will be referred to the appropriate treatment or support service and an appropriate package of care and/or support will be instigated.
