As a Health Promotion Officer for the Your Health
Notts Integrated Wellbeing Service, you will work alongside communities to
improve health outcomes through education, engagement and community
development.
Working across targeted former mining communities,
you will build trusted relationships with residents, voluntary organisations,
community groups, health professionals and partner agencies to improve
awareness of local health services and support people to access appropriate
wellbeing interventions.
You will support individuals and communities to
increase their understanding of health and wellbeing, helping to build
confidence, improve health literacy and encourage positive lifestyle changes.
As part of your professional development, you will complete the RSPH Level 2
Award in Understanding Health Improvement (if not already achieved), equipping
you with the knowledge and skills to promote health improvement within local
communities.
Using a strengths-based, asset-based community
development approach, you will identify local assets, listen to community
priorities and work collaboratively with local people to reduce health
inequalities and improve access to preventative health services.
This is a varied and rewarding role requiring
flexibility, initiative and excellent communication skills. No two days will be
the same, with opportunities to engage with local communities, attend community
events, support local projects and work alongside colleagues across the
Integrated Wellbeing Service.
You will receive support from ABL Health's
experienced multidisciplinary team, including health professionals, community
engagement specialists, service development, business intelligence, governance,
finance and operational teams.
Duties and
Responsibilities
This post will work under the line management of
the System Integration Lead . The role will be varied, rewarding and
community-focused, supporting the implementation, delivery and ongoing
development of health promotion initiatives across Nottinghamshire.
Key
Responsibilities
Promote health and wellbeing across Nottinghamshire through
community-based health promotion activities.
Work within targeted former mining communities to reduce health
inequalities and improve health outcomes.
Build and maintain positive, trusted relationships with residents,
community groups, voluntary organisations, faith groups, local authorities,
Primary Care Networks, GP practices and other partner organisations.
Act as a trusted link between local communities and health services,
supporting individuals to access appropriate health and wellbeing services and
reducing barriers to engagement.
Promote the Integrated Wellbeing Service through community outreach,
engagement events, awareness campaigns and partnership working.
Increase awareness of local health services and encourage referrals into
smoking cessation, weight management, alcohol reduction, physical activity and
falls prevention programmes.
Support individuals and communities to improve their health literacy,
confidence and understanding of healthy lifestyle behaviours.
Undertake the RSPH Level 2 Award in Understanding Health Improvement as
part of your induction and ongoing professional development (if not already
achieved).
Work collaboratively with community partners to identify local health
priorities and develop appropriate health promotion activities.
Support community capacity building by identifying local strengths,
assets and opportunities to improve health and wellbeing.
Encourage co-production by involving local residents in the design,
development and evaluation of health promotion initiatives.
Attend community events, local forums, partnership meetings and
networking opportunities to represent the Integrated Wellbeing Service.
Work closely with colleagues across the Integrated Wellbeing Service to
ensure seamless support and appropriate referrals.
Collect monitoring information, community feedback and outcome data to
support service evaluation and continuous improvement.
Maintain accurate records and contribute to reporting requirements in
line with organisational policies.
Support communication and engagement activities, including awareness
campaigns, promotional materials and social media where appropriate.
Demonstrate innovation and creativity when developing new approaches to
engaging communities and reducing health inequalities.
Work flexibly across Nottinghamshire, including occasional evening or
weekend events where required.
Support colleagues across the wider service when required to ensure
high-quality service delivery.
Skills and
Competencies Required
Experience of working within community health, public health, health
improvement, social prescribing, community development or a related field.
Experience of engaging effectively with local communities and building
trusted relationships.
Understanding of health inequalities and the wider determinants of
health.
Knowledge of community engagement approaches and asset-based community
development principles.
Willingness to undertake and successfully complete the RSPH Level 2
Award in Understanding Health Improvement as part of your induction and ongoing
professional development (or already hold an equivalent qualification).
Excellent communication, interpersonal and relationship-building skills.
Ability to communicate effectively with people from a wide range of
backgrounds and adapt communication to meet individual needs.
Ability to motivate, empower and support individuals to make positive
changes to their health and wellbeing.
Experience of working collaboratively with partner organisations and
multidisciplinary teams.
Excellent organisational skills with the ability to manage competing
