Job description

To manage a caseload of clients with complex needs, using evidence based/client centred principles to assess, plan, implement and evaluate interventions. To carry out joint OT assessments to support the physical health OTs when required. To provide a variety of interventions including direct clinical work with patients, support for families and carers and consultation across the wider professional networks. To provide short-term follow up care for patients in their own homes when required. To participate in the planning, development and evaluation of OT services within the identified service, holding responsibility for defined projects. To contribute to the maintenance and development of the Trust OT Service. To regularly supervise OT students on practice placement. To promote Recovery & Social Inclusion. To act as the designated Disability Champion, leading the multi-disciplinary team in Disability Equality matters within the service and other areas as required.
