Job description

The Nursing Associate will be an integral member of the Community Mental Health Team, supporting the Community Non-Medical Prescriber (NMP) Nurse and multidisciplinary team to deliver safe, effective, high-quality care. They will contribute to medication oversight, physical health monitoring, health promotion, and person-centred care for service users with complex mental and physical health needs. Working under the supervision of registered practitioners, the post holder will undertake physical health assessments, monitor and report clinical observations, support medicines management, and contribute to care plans. They will work with service users, carers, primary care, and partner agencies to promote holistic care, identify physical health concerns early, and address medication-related issues. The role supports improved physical wellbeing through effective monitoring and interventions, in line with Trust policies, national guidance, and professional standards. £750 Refer a Friend Scheme – Know someone who would be a great addition to our team? Financial support may be available in line with the Trust's Relocation Policy to help you make the move and settle into your new role |Support the Community NMP Nurse in the delivery of medication oversight and medicines management processes.Undertake routine physical health monitoring, including blood pressure, pulse, weight, BMI, blood glucose monitoring, ECGs and venepuncture where competent.Identify and report physical health concerns, medication side effects and clinical deterioration to the registered practitioner.Support the monitoring requirements associated with psychotropic medications in accordance with local and national guidance.Assist in the coordination and delivery of physical health clinics and community-based health interventions.Promote healthy lifestyles and provide health education relating to physical health, medication adherence and wellbeing.
