Job Summary The Nurse Practitioner (NP) is an advanced practice registered nurse responsible for providing high-quality, patient-centred healthcare services. The NP assesses, diagnoses, treats, and manages acute and chronic health conditions, promotes health and wellbeing, and collaborates with multidisciplinary healthcare teams to deliver comprehensive care. The role includes conducting clinical examinations, ordering and interpreting diagnostic tests, prescribing medications within scope of practice, and educating patients on disease prevention and management. Key Responsibilities Clinical Care • Conduct comprehensive patient assessments, including medical histories and physical examinations. • Diagnose and manage acute and chronic illnesses. • Develop, implement, and evaluate treatment plans. • Prescribe medications and therapies in accordance with professional guidelines and regulatory requirements. • Order, interpret, and act upon laboratory and diagnostic test results. • Provide urgent, routine, and preventive care services. Patient Education and Support • Educate patients and families regarding diagnoses, treatment options, and self-management strategies. • Promote health maintenance, disease prevention, and healthy lifestyle choices. • Support patients in making informed healthcare decisions. Care Coordination • Collaborate with physicians, nurses, pharmacists, and other healthcare professionals. • Coordinate referrals to specialists and community services as required. • Participate in multidisciplinary team meetings and case reviews. Documentation and Compliance • Maintain accurate, timely, and confidential patient records. • Ensure compliance with professional, legal, regulatory, and organisational standards. • Adhere to safeguarding, infection prevention, and clinical governance requirements. Quality Improvement • Contribute to service development and quality improvement initiatives. • Participate in audits, research activities, and evidence-based practice projects. • Support the implementation of clinical policies and procedures.
