Clinical: • Undertake home visits as assessed and when appropriate • Assessment and triage of acute presentations including telephone triage • Contributing towards the running a minor illness/ injury walk-in service • Assisting in the training of frontline staff • Make direct referrals to primary, secondary and social services within locally • agreed pathways, guidance and protocols • ECG and ABPM interpretation • Assisting with other ad hoc clinical tasks • Ensure complete and accurate documentation of every patient contact Professional: • Participate in an annual individual performance review, taking responsibility for maintaining a record of own personal and professional development • Undertake professional development as required by the role to ensure thatup-to-date evidence-based knowledge and competence in all aspects of the role is maintained • Monitor the effectiveness of your clinical practice through quality andassurance strategies such as the use of audit and peer review • Participate in Practice Meetings • Adhere to practice policies, procedures and guidelines • Diagnosing and treating patients presenting with minor illness • Triage and treat patients wishing to see a health care professional, making anynecessary referrals to other members of the primary health care team • Patients with problems needing referral to secondary care should be discussed with the registered GP before making such referral. • Ensure clinical practice is safe and effective and remains within boundaries ofcompetence, and to acknowledge limitations. • Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary • Works from the surgery and within communities as an autonomous practitioner caring for patients and families. • Works as an autonomous practitioner, in accordance with the Health Professions Council.
