Paramedic Practitioner

• Working autonomously but with full clinical support, you will manage patients presenting with a wide range of minor illness and injury, as well as long-term conditions where appropriate • You will assess, diagnose, treat, refer or signpost patients who attend the surgery • Provide clinical assessment, diagnosis, treatment, and evaluation of patients • Work collaboratively with GPs, nurses, pharmacy Technician and admin team • Support the management of long-term conditions and care planning • Support and participate in shared learning across the practice • Assess patients presenting with acute illness or for review of chronic disease conditions • Undertake home visits as required • Clinically examine and assess patient needs from a physiological and psychological perspective and plan care accordingly • Advise patients on general healthcare and promote self-management where appropriate, including signposting to other community or voluntary services • Communicate at all levels across organisations ensuring that an effective, patient-centred service is delivered. • Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required. • Maintain accurate health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice. • Independent prescribing. We will consider applications from Paramedic Practitioners who have not yet achieved this, they would be supported to do so in time.
