Job description
To undertake early discharge of elective and emergency patients, utilising clinical skills, such as consultation, assessment and diagnostic skills and non medical prescribing. Discharge is within a set discharge criteria facilitating early discharge and virtual monitoring following admission, in turn improving the flow of patients requiring treatment and achieving safe discharge. This will include assessment and planning of ongoing care. Liaising with teams across Essex to ensure appropriate follow up is provided to these patients.
To manage a caseload of high-risk patients who have undergone PCI in the setting of acute coronary syndrome (ACS) and elective patients awaiting complex percutaneous coronary procedures, utilising clinical consultation, assessment, diagnostic skills and non medical prescribing practice to manage the patients condition. Any changes to the management plan are shared with the GP and wider MDT if appropriate to ensure continuity and a high standard of evidence-based care, from point of assessment through to PCI treatment, medical management, discharge and personalised follow up.
Work autonomously managing their own caseload of patients, whilst working as part of the multi-disciplinary team. Ensuring continuity and a high standard of evidenced based care, from point of assessment through to treatment, management, discharge from hospital and follow-up in outpatients clinic.
Facilitate, prepare and lead on the monthly complex PCI meeting. Monthly planning, organisation and presentation for this meeting. This requires history taking and assessment of symptoms of patients referred for complex PCI. Also includes facilitation of the monthly meeting, taking minutes, managing outcomes, and ensuring the patient and referrer are notified of the management plan.
Participation in specialist education and training for staff across the Essex Cardiothoracic Centre (ECTC) and wider cardiology cohort. Undertake training and education of student nurses, new staff and junior doctors. Presentation of a range of topics in relation to PCI and data collection for the service.
Undertake weekly telephone Preassessment Clinic. Identifying any barriers to a safe, well managed admission to hospital for elective PCI. Ensuring that medical and social factors are managed pre admission to ensure safe admission.
