Job description

The post holder will Be an experienced RGN/RMN with considerable knowledge of dementia care. NMC registration required. Perform biopsychosocial specialist nursing assessments to support a large caseload of families affected by dementia with complex needs from peri diagnosis to the end of life Facilitate improvements in end of life care for people with dementia through the development of advance care plans Provide inspirational professional leadership and direction for the delivery of high-quality dementia care Establish excellent and seamless pathways of care for patients living with dementia and their carers Apply the Admiral Nurse Competency framework at an enhanced specialist level including person-centred care, therapeutic skills, and triadic relationships, sharing knowledge, best practice and critical reflective practice Promote the physical, social and psychological health and well-being of carers and people with dementia, including understanding the interaction between dementia and other co-morbidities Supporting carers and families through the complexity surrounding loss, transitions and changing relationships Act as an expert and collaborative advanced practitioner who will provide advice, education and support for people with dementia, families and members of the multidisciplinary healthcare team Support the delivery of key quality and performance indicators agreed by Dementia UK and IMP Primary Care Network Job description Job responsibilities The post holder will Be an experienced RGN/RMN with considerable knowledge of dementia care.
