PCN Nurse

Job Purpose The post holder will work as part of the South Sefton Primary Care Network (PCN) multidisciplinary team (MDT) to deliver high-quality, safe, and patient-centred care across community and primary care settings. The role will focus on the delivery of proactive, planned care, with a particular emphasis on: • Chronic disease management for housebound patients • Learning Disability (LD) annual health checks in line with national requirements The post holder will also contribute to the development of the nursing workforce by supporting, supervising, and mentoring Student Nurse Associates and Nursing Associates, fostering a positive learning and development environment within the PCN. Key Duties and Responsibilities Chronic Disease Management (Housebound Patients) Undertake comprehensive annual reviews for housebound patients with long-term conditions. Complete holistic patient assessments, including clinical observations, symptom review, medication concordance, and lifestyle assessment. Develop, implement, and evaluate personalised care plans in partnership with patients and carers. Identify clinical deterioration, emerging risks, or unmet needs and escalate appropriately within the MDT. Ensure accurate, contemporaneous, and legally compliant clinical documentation. Learning Disability (LD) Annual Health Checks Deliver LD annual health checks in accordance with national DES requirements and best practice guidance. Work collaboratively with patients, families, carers, and support providers to ensure person-centred care. Identify and address reasonable adjustments, safeguarding concerns, and health inequalities. Develop and support the implementation of Health Action Plans and coordinate appropriate follow-up care. Promote preventative healthcare, including screening programmes and women's health initiatives. Supervision, Education and Mentorship Act as a professional role model, demonstrating high standards of clinical practice. Provide appropriate day-to-day supervision and support to Student Nurse Associates and Nursing Associates within scope of practice. Contribute to induction programmes, teaching sessions, and ongoing staff development. Identify and escalate concerns regarding competence, conduct, or patient safety in line with local policies. Professional Practice and Governance Maintain active NMC registration and fulfil all revalidation requirements. Practice in accordance with the NMC Code and relevant national and local policies, procedures, and guidelines. Adhere to infection prevention and control standards and ensure safe clinical practice at all times. Promote safeguarding for both adults and children, recognising and responding to concerns appropriately. Champion equality, diversity, and inclusion in all aspects of care delivery. Participate in clinical audit, service evaluation, and quality improvement initiatives to enhance patient outcomes. Communication and MDT Working Communicate effectively, sensitively, and professionally with patients, carers, and colleagues. Work collaboratively within the PCN MDT, including GPs, Advanced Nurse Practitioners, Care Coordinators, and Allied Health Professionals. Utilise clinical systems effectively to ensure accurate documentation, coding, and information sharing in line with data protection requirements.
