Key Responsibilities
Service Coordination and Administration
• Provide administrative support across PCN services and projects.
• Coordinate appointments, clinics, meetings, training sessions and patient activities.
• Manage diaries and schedules for PCN staff and clinicians as required.
• Support the booking, cancellation and rebooking of patient appointments.
• Assist with managing patient communications through telephone, email and digital systems such as Accurx.
• Act as a first point of contact for service enquiries, responding to requests or directing them to the appropriate team.
Patient and Referral Management
• Process referrals and ensure they are allocated to the appropriate service or team.
• Support clinicians and care teams to manage patient caseloads and pathways.
• Monitor missed appointments (DNAs) and help rebook or follow up patients when appropriate.
• Maintain accurate and confidential patient records in line with NHS policies and data protection requirements.
Supporting PCN Services
• Help maintain clinical systems, appointment books and clinic templates to support the smooth running of PCN services.
• Support a range of services including respiratory, dermatology, home visiting, phlebotomy, musculoskeletal and enhanced access clinics.
• Assist with service administration and the monitoring of activity across PCN programmes.
Project and Team Support
• Assist with the delivery of PCN projects, events and health promotion initiatives.
• Support audits, service reviews and quality improvement activities.
• Prepare meeting agendas, take notes and maintain action logs when required.
• Support the coordination of staff, training and stakeholders involved in PCN activities.
• Assist with administrative tasks linked to patient surveys, searches and communications.
Operational Support
• Monitor and order clinical and non-clinical supplies to ensure services have the resources they need.
• Report any issues relating to stock, equipment or service delivery.
• Support colleagues across the wider PCN team and provide cover during periods of leave when required.
• Escalate any concerns identified from reports or service activity in line with PCN policies and procedures.
