Provide holistic, evidence-based care for multi-pathology patients on Rochester West Ward, making clinical diagnoses and decisions within the scope of practice, based on strong clinical examination skills, evidence-based practice, and Trust protocols. Clinically assess patients, ensuring safety and quality of care, and escalate or de-escalate medical oversight as appropriate. Utilise appropriate bedside tests and available investigations in combination with extended clinical skills to support the assessment, diagnosis, and ongoing management of patients in the rehab/intermediate care setting. Develop and maintain effective working relationships with all members of the multidisciplinary team to promote continuity, communication, and quality of care. Support the development and quality improvement of the rehabilitation service, contributing ideas, feedback, and clinical insight to enhance patient outcomes and service efficiency.
