Some of the key responsibilities of the job: • Assess, plan and manage care for patients with long-term conditions, promoting self-management and healthy lifestyles. • Deliver vaccination and immunisation programmes, including travel vaccinations where appropriate. • Undertake cervical screening, wound care and other routine practice nursing procedures. • Provide health promotion, screening and preventative care to improve patient outcomes. • Maintain accurate clinical records and ensure care is delivered in line with professional standards and best practice.
