Job description

Job responsibilities: Work as part of the Nursing team to assess, plan, provide and implement and evaluate nursing care to meet the care needs of individuals in the practice population including assessment, planning, delivery and evaluation of: • Wound management including Post-operative wound care • Ear irrigation • Immunisation/injection including childhood and basic travel • Spirometry/respiratory testing • Cervical cytology • Ear syringing • Blood pressure and other physiological measurements • Venepuncture • Long Term conditions annual reviews Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition Assist in and perform routine tasks related to patient care as directed by GPs Working alongside the GPs to manage Chronic Disease registers including Asthma, COPD, Diabetes, Heart Disease etc. Maintain disease registers in liaison with administrative staff. Undertake annual review of patients understanding and ability to self-manage.
